The metal tier health plans (Bronze through Platinum) are known as Core PPO Plans. Their Minimum Coverage plan is an EPO. These plans will be offered through the Marketplace exchange of Covered California and off-exchange directly from United Healthcare (UHC). UHC will only be offering standard benefit design plans established by Covered California including one Bronze Health Savings Account.
