Oscar California 2016 health plan documents

Oscar_fitness_trackerOscar has posted plan specific documents relating to their California individual and family on and off exchange plans. These documents will help educate consumers about the Oscar health plans. In particular, consumers should carefully review and understand Oscar’s Simple and Simple Plus series of non-standard, off-exchange health plans quite different from the standard benefit design. Oscar will be offering plans in Region 16, Southwest Los Angeles County, and Region 18, Orange County.

Plan documents released by Oscar for open enrollment beginning November 1, 2015 are

  • Evidence of Coverage
  • Summary of Benefits and Coverage
  • Plan overview of standard benefit design and non-standard off-exchange plans
  • Disabled Dependent Form
  • HIPAA Authorization Form
  • Members Rights and Responsibilities
  • Drug formulary
  • Participating hospitals
  • Paper applications for both open enrollment and special enrollment periods

Oscar offers health plans that conform to the standard benefit design approved for purchase through Covered California. These standard benefit design plans are eligible for the tax credit subsidy through Covered California or may be purchased directly from Oscar with no subsidy. Like many health insurance companies, Oscar also offers non-standard benefit design plans that are only available from Oscar directly. Oscar’s off-exchange plans consist of the Simple and Simple Plus series.

For consumers who enroll in an Oscar health plan, either through Covered California or off-exchange directly from Oscar, members can receive a free Misfit Flash to track their steps. With the Misfit Flash linked to a member’s smart phone, every day they hit their goal they are eligible to receive a $1 credit on an Amazon gift card.
