Cigna to require proof of identity, residency and payment for health insurance

Cigna has announced they will be sending letters out to individuals newly enrolled in their health plans in 2016. The letters requesting verification of identity, residency, and premium payment will be sent to Cigna Individual and Family health plan enrollments regardless of whether the plan was purchased either through a Market Place Exchange or off-exchange directly from Cigna. If Cigna doesn’t receive the requested information within 45 days the health insurance may be cancelled.

There are three different letters verification letters that Cigna Individual and Family Plan members may receive.

  1. Letter requesting verification of identity and proof of premium payment
  2. Letter requesting verification of identity, residency, and proof of premium payment
  3. Letter requesting verification of residency and proof of premium payment

Letters requesting identity verification will require at least documents to confirm the person’s identity. Valid forms of proof include

  •  Valid driver’s license/non-driver ID card;
  •  Social Security Card;
  •  Passport;
  •  Permanent Resident Card;
  •  Work Visa; or
  •  Student Visa

The residency or home address verification will require at least two documents in some instances to establish that the person on the health plan has resided at the address for at least 30 days. Valid forms of residency verification can include

  •  Up to date driver’s license/learner permit/non-driver ID card*;
  •  Passport* or passport card*;
  •  Voter registration form*;
  •  Utility bill or credit card statement, issued in the past 90 days;
  •  Bank statement or cancelled check (with preprinted name and address) dated within the past 90 days;
  •  Checking/savings account statement, issued in the past 90 days;
  •  Property tax bill/receipt for the current year; or
  •  Up to date homeowners/renters insurance policy.

All verification letters will ask for proof of premium payments. Cigna is working to insure that certain third party entities are not paying the premiums on behalf of certain enrolled members. Only someone who is making their own premium payments or having a close friend or relative will generally have access to cancelled checks. Some third party payers may be paying the premiums for some plan members in order to direct them to their facilities for treatment or procedures. Acceptable documentation for validating premium payments include

  • Copy of both sides of a cancelled check;
  • Credit card statement;
  • Bank account statement.

Cigna plan member or their authorized representatives will also have to sign a Certification of True Copy indicating who actually pays the premiums and that the documentation is true and correct.

«Customer_State» Individual & Family Plan Verification – We Need You To Respond

Dear «Customer_First_Name»:

Thank you for applying for an Individual and Family Plan through Cigna. We’re writing to let you know that we need some more information from you to finish processing your application. Specifically, we need you to confirm your home (residency) address. We also need to know how you’ll pay your premiums.

Why do I need to give Cigna this information?

In 2010 the Patient Protection and Affordable Care Act (ACA) became law. The ACA limits enrollment in qualified health plans to people who meet certain criteria. This means that we need more information to approve your application.

What information do I need to send?

Residency/Home Address Verification

All together we need you to send us at least two documents. One should show your home (residential) address as of your enrollment date [insert date]. The other one should show your home address within the last 30 days. At least one type of document should be a government issued form of identification. This means that we need either the enrollment date document or the 30 days document to be a government ID. Examples of documents you can use include:

  •  Up to date driver’s license/learner permit/non-driver ID card*;
  •  Passport* or passport card*;
  •  Voter registration form*;
  •  Utility bill or credit card statement, issued in the past 90 days;
  •  Bank statement or cancelled check (with preprinted name and address) dated within the past 90 days;
  •  Checking/savings account statement, issued in the past 90 days;
  •  Property tax bill/receipt for the current year; or
  •  Up to date homeowners/renters insurance policy.

Premium Payment The U.S. Department of Health and Human Services instructs insurance companies to reject premium payments received from certain third parties on behalf of individual customers. Please send proof showing that you paid your insurance premium or identify the party that has paid on your behalf. Acceptable proof of payment documentation showing your premium payment includes one of the following:

  •  Copy of both sides of a cancelled check;
  •  Credit card statement;
  •  Bank account statement.

Identity Verification We need to validate your identity as of your enrollment with Cigna on [date]. Please send a copy of at least two of the following documents. Acceptable documents include:

  •  Valid driver’s license/non-driver ID card;
  •  Social Security Card;
  •  Passport;
  •  Permanent Resident Card;
  •  Work Visa; or
  •  Student Visa.

It’s important that we get this information as soon as possible. Please send us the documents above and the attached Certification of True Copy form within 45 days.

What happens if I do NOT give Cigna the information?

If we do not get the requested information, we’ll need to cancel your coverage. We also won’t process any future claims. We may also have to ask your doctors and other healthcare professionals to refund us any payments we made for claims already paid.

Please send us this required information with the attached Certification of True Copy form within 45 days. You can either mail or fax us your documents with the completed form. If you choose mail, a prepaid envelope is enclosed for you to use. If you prefer, you may fax your information to us using our secure line at 866.463.3075.
