New Blue Shield List Bill option for employees

Employer Deductions, List Billing: A New Option
If your client can’t afford coverage or wants to cancel their Small Business health plan in 2016, you can offer another option for their employees to have access to quality Blue Shield coverage.
Called List Billing, it begins with you helping their employees choose on-exchange or off-exchange Individual and Family Plans (IFP).
When a minimum of three employees are enrolled in IFP plans, you can offer List Billing by having the employer and employees complete and submit the appropriate forms:
List Bill Transmittal Form – this form must be submitted with the appropriate Agreement for Payment Form and bank account document signed and attached. The transmittal form is required to initiate a List Bill for new applicants or to add an existing Blue Shield subscriber to a List bill that is already in place.
Agreement for Payment Form – this form must be submitted for every subscriber 18 years or older who wishes to have their dues paid via the List Bill option. Please ensure an adult subscriber signs this document. Please copy the original agreement form that is provided as needed for each subscriber.
Producer of Record Change Form – this form must be completed if the agent for the List Bill is not the current agent of record for an existing subscriber. The form authorizes the agent to access information on the subscriber and make payment inquiries as the servicing agent.
We will set up List Billing for the employer who will then receive automatic monthly post-tax premium deductions from the paychecks of each IFP-health-plan-enrolled employee.
This List Billing service will be available for February 1, 2016 bill dates. Enrollment into the free List Billing service is effective November 1, 2015.
