2016 L.A. Care Covered Health Plans

L.A. Care Covered will offer standard benefit design health plans in all of Region 16 (S.W. Los Angeles County) and part of Region 15 (N.E. Los Angeles County). All their plans are HMOs and they offer no non-standard benefit design plans off the exchange.

In 1997 L.A. Care HMO (health maintenance organization) was established  as a locally created health plan to provide health care services to Medi-Cal beneficiaries under California’s Two-Plan Model. Today, L.A. Care serves all of Los Angeles County with their HMO plans for Medi-Cal, PASC-SEIU Homecare Workers, Medicare Advantage and ACA compliant individual and family plans offered through Covered California. L.A. Care Covered is their portfolio of individual and family plans that can be purchased directly from L.A. Care.

2016 Evidence of Coverage and Member Handbook

This publication is called a Subscriber Agreement & Combined Evidence of Coverage and Disclosure Form (also called the Subscriber Agreement & Member Handbook).
